TERMS & AGREEMENT

 

INTRODUCTION

Congratulations on your decision to participate in our exercise and nutrition program! With the help of your personal trainer(s), you greatly improve your ability to accomplish your training goals faster, safer, and with maximum benefits. The details of these training sessions can be used for a lifetime. In order to maximize progress, it will be necessary for you to follow program guidelines during supervised and (if applicable) unsupervised training days. Remember, exercise and healthy eating are EQUALLY important! During your exercise program, every effort will be made to assure your safety. However, as with any exercise program, there are risks, including increased heart stress and the chance of musculoskeletal injuries. In volunteering for this program, you agree to assume responsibility for these risks and waive any possibility for personal damage. You also agree that, to your knowledge, you have no limiting physical conditions or disability that would preclude an exercise program. By signing below, you accept full responsibility for your own health and well-being AND you acknowledge an understanding that no responsibility is assumed by the leaders of the program. It is recommended that all program participants work with their personal trainer three (3) times per week. However, due to scheduling conflicts and financial considerations, a combination of supervised and unsupervised workouts is possible.

TRAINERS OBLIGATION

The trainer(s) will use his/her skills and knowledge to design a safe program of exercise that will take into account your lifestyle, personal goals, fitness levels and medical history.
The trainer(s) will provide the coaching, supervision, advice and support that you will need to achieve your goals. Apart from the initial consultation (and the first personal training session if taken immediately afterwards) each personal training session will last 30 minutes (a “Session”).
You understand that the results of any fitness program cannot be guaranteed. Your progress depends on your effort and co-operation in and outside of the sessions. In particular you acknowledge that individual results may vary and no particular result is guaranteed by your trainer(s).

CLIENTS OBLIGATION

It is understood between you and your trainer(s), both must commit to your training program 100% in order for you to achieve results.
You are required to arrive on time for each session so that the trainer’s full training plan is achieved on each visit.
You are required to wear appropriate clothing and footwear. If you do not wear proper clothing and/ or footwear, you understand that the trainer has the option to not providing training service until you have proper clothing and/ or footwear.
You are required to complete a Liability Waiver before undertaking your first personal training session.
Your trainer(s) may require a letter of ‘medical clearance’ from your GP. Please be aware that your GP may charge for providing this letter.
You understand and agree that it is your responsibility to inform the trainer(s) of any conditions or changes to your health, now and ongoing, which might affect your ability to exercise safely and with minimal risk of injury. If your trainer(s) require further medical information from a practitioner, you must provide such details.
You understand that there are inherent risks in participating in a program of strenuous exercise. If you sustain or claim to sustain any injury while participating in training, you acknowledge that the trainer(s) is not responsible, except where the injury was caused by his/her gross negligence or intentional act.
Your trainer(s) cannot be held liable in any way for undeclared or unknown medical conditions.

 PAYMENT POLICY

You understand that certain programs including monthly payments are automatically re-occurring every thirty (30) days until you give notice that you have decided to discontinue future training services.

You understand no personal training REFUNDS will be issued for any reason, including but not limited to relocation, illness, and unused sessions.

You understand that you can cancel future re-occurring payments at any time without charge.

LATE POLICY

If the client is late the session cannot be extended and will end at the appointed time.
If the trainer is late additional time will be added to the session or to subsequent sessions.

CANCELLATION POLICY

You understand that 6 hours notice of cancellation or re-schedule is required for all appointments. If session is not cancelled in 6 hours or more, you understand that the training session cannot be made up. However there is not a cancellation fee.

TRAINING SESSIONS

You understand that by purchasing the semi-private program, you may train with other clients during training sessions.

You understand that by purchasing a private training program, you will train one on one during all training sessions.
